The California Department of Public Health (CDPH) reviewed 109 cases of healthcare personnel (HCP) with laboratory-confirmed mpox to understand transmission risk in healthcare settings. Overall, 90% of HCP with mpox had nonoccupational exposure risk factors. One occupationally acquired case was associated with sharps injury while unroofing a patient’s lesion for diagnostic testing.
“It Is A Carrot-Stick Model”: A Qualitative Study of Rural-Serving Clinician and Rural-Residing Veteran Perceptions of Requirements to Quit Smoking prior to Elective Surgery

Introduction. Some medical centers and surgeons require patients to stop smoking cigarettes prior to elective orthopaedic surgeries in an effort to decrease surgical complications. Given higher rates of smoking among rural individuals, rural patients may be disproportionately impacted by these requirements. We assessed the perceptions and experiences of rural-residing Veterans and clinicians related to this requirement. Methods. We conducted qualitative semistructured one-on-one interviews of 26 rural-residing veterans, 10 VA orthopaedic surgery staff (from two Veterans Integrated Services Networks), 24 PCPs who serve rural veterans (14 VA; 10 non-VA), and 4 VA pharmacists. Using the knowledge, attitudes, and behavior framework, we performed conventional content analysis. Results. We found three primary themes across respondents: (1) knowledge of and the evidence base for the requirement varied widely; (2) strong personal attitudes toward the requirement; and (3) implementation and possible implications of this requirement. All surgery staff reported knowledge of requirements at their institution. VA PCPs reported knowledge of requirements but typically could not recall specifics. Most patients were unaware. The majority of respondents felt this requirement could increase motivation to quit smoking. Some PCPs felt a more thorough explanation of smoking-related complications would result in increased quit attempts. About half of all patients reported belief that the requirement was reasonable regardless of initial awareness. Respondents expressed little concern that the requirement might increase rural-urban disparities. Most PCPs and patients felt that there should be exceptions for allowing surgery, while surgical staff disagreed. Discussion. Most respondents thought elective surgery was a good motivator to quit smoking; but patients, PCPs, and surgical staff differed on whether there should be exceptions to the requirement that patients quit preoperatively. Future efforts to augment perioperative smoking cessation may benefit from improving coordination across services and educating patients more about the benefits of quitting.

Background.
Compulsory admission procedures of patients with mental disorders vary between countries in Europe. The Ethics Committee of the European Psychiatric Association (EPA) launched a survey on involuntary admission procedures of patients with mental disorders in 40 countries to gather information from all National Psychiatric Associations that are members of the EPA to develop recommendations for improving involuntary admission processes and promote voluntary care.
Methods.The survey focused on legislation of involuntary admissions and key actors involved in the admission procedure as well as most common reasons for involuntary admissions.
Results.We analyzed the survey categorical data in themes, which highlight that both medical and legal actors are involved in involuntary admission procedures.
Conclusions.We conclude that legal reasons for compulsory admission should be reworded in order to remove stigmatization of the patient, that raising awareness about involuntary admission procedures and patient rights with both patients and family advocacy groups is paramount, that communication about procedures should be widely available in lay-language for the general population, and that training sessions and guidance should be available for legal and medical practitioners. Finally, people working in the field need to be constantly aware about the ethical challenges surrounding compulsory admissions.
The relationship between antihypertensive medications and mood disorders: analysis of linked healthcare data for 1.8 million patients

Background
Recent work suggests that antihypertensive medications may be useful as repurposed treatments for mood disorders. Using large-scale linked healthcare data we investigated whether certain classes of antihypertensive, such as angiotensin antagonists (AAs) and calcium channel blockers, were associated with reduced risk of new-onset major depressive disorder (MDD) or bipolar disorder (BD).
MethodTwo cohorts of patients treated with antihypertensives were identified from Scottish prescribing (2009–2016) and hospital admission (1981–2016) records. Eligibility for cohort membership was determined by a receipt of a minimum of four prescriptions for antihypertensives within a 12-month window. One treatment cohort (n = 538 730) included patients with no previous history of mood disorder, whereas the other (n = 262 278) included those who did. Both cohorts were matched by age, sex and area deprivation to untreated comparators. Associations between antihypertensive treatment and new-onset MDD or bipolar episodes were investigated using Cox regression.
ResultsFor patients without a history of mood disorder, antihypertensives were associated with increased risk of new-onset MDD. For AA monotherapy, the hazard ratio (HR) for new-onset MDD was 1.17 (95% CI 1.04–1.31). Beta blockers' association was stronger (HR 2.68; 95% CI 2.45–2.92), possibly indicating pre-existing anxiety. Some classes of antihypertensive were associated with protection against BD, particularly AAs (HR 0.46; 95% CI 0.30–0.70). For patients with a past history of mood disorders, all classes of antihypertensives were associated with increased risk of future episodes of MDD.
ConclusionsThere was no evidence that antihypertensive medications prevented new episodes of MDD but AAs may represent a novel treatment avenue for BD.

In contrast to much of the political economy literature, this article explores acts of refusal that obstruct attempts to impose austerity measures on advanced industrial democracies. It thereby complements a literature that has thus far focused far more upon the (apparently unobstructed) imposition of austerity. In doing so, it uses two typically ‘low-resistance’ countries – Japan and the UK –as least-likely cases and finds that austerity is rarely uncontested. Using fuzzy set Qualitative Comparative Analysis, it highlights the ‘causal recipes’ sufficient for both (1) anti-austerity activity to have a significant impact on austerity proposals and (2) the smooth (unobstructed) imposition of austerity. The politics of austerity is shown to be better understood as an iterative interaction between proposals for austerity and the acts of refusal they encounter. These obstacles to austerity appear more straightforward to activate effectively in Japan’s coordinated model of capitalism, whilst the UK’s liberal market economy tends to generate more innovative forms of dissent that (if they are sufficiently militant) provide an alternative route towards the obstruction of austerity.

Serum and erythrocyte (RBC) total folate are indicators of folate status. No nationally representative population data exist for folate forms. We measured the serum folate forms (5-methyltetrahydrofolate (5-methylTHF), unmetabolised folic acid (UMFA), non-methyl folate (sum of tetrahydrofolate (THF), 5-formyltetrahydrofolate (5-formylTHF), 5,10-methenyltetrahydrofolate (5,10-methenylTHF)) and MeFox (5-methylTHF oxidation product)) by HPLC–MS/MS and RBC total folate by microbiologic assay in US population ≥ 1 year (n approximately 7500) participating in the National Health and Nutrition Examination Survey 2011–2. Data analysis for serum total folate was conducted including and excluding MeFox. Concentrations (geometric mean; detection rate) of 5-methylTHF (37·5 nmol/l; 100 %), UMFA (1·21 nmol/l; 99·9 %), MeFox (1·53 nmol/l; 98·8 %), and THF (1·01 nmol/l; 85·2 %) were mostly detectable. 5-FormylTHF (3·6 %) and 5,10-methenylTHF (4·4 %) were rarely detected. The biggest contributor to serum total folate was 5-methylTHF (86·7 %); UMFA (4·0 %), non-methyl folate (4·7 %) and MeFox (4·5 %) contributed smaller amounts. Age was positively related to MeFox, but showed a U-shaped pattern for other folates. We generally noted sex and race/ethnic biomarker differences and weak (Spearman's r< 0·4) but significant (P< 0·05) correlations with physiological and lifestyle variables. Fasting, kidney function, smoking and alcohol intake showed negative associations. BMI and body surface area showed positive associations with MeFox but negative associations with other folates. All biomarkers showed significantly higher concentrations with recent folic acid-containing dietary supplement use. These first-time population data for serum folate forms generally show similar associations with demographic, physiological and lifestyle variables as serum total folate. Patterns observed for MeFox may suggest altered folate metabolism dependent on biological characteristics.

The insertion of nanostructured materials (such as quantum wells, wires, and dots) into the intrinsic region of p-i-n solar cells introduces an intermediate band within the bandgap of the host material. It has been shown that the sub-bandgap conversion provided by the nanostructured materials, enhances the short circuit current as well as the overall efficiency of InAs quantum dots (QD) imbedded in GaAs superlattice (SL) solar cells [1]. As a contender for space applications, it is necessary to subject these solar cell structures to temperatures encountered in the Low Earth Orbit (LEO), probing for any material degradation. Herein, we focus on temperature dependent characterization using high resolution X-ray diffraction (HRXRD) of InAs QD enhanced GaAs solar cell structures with varying growth parameters. The structures characterized can be classified into three groups: (1) GaP strain compensation coverage, (2) GaAs barrier coverage, and (3) InAs coverage for QD formation. HRXRD rocking curves of each structure focusing around the GaAs peak are analyzed at a range of temperatures up to 200˚C. Although no noticeable shifts in the SL peaks are detected, interfacial diffusion decreased the resolution of fringes produced by reflections at the SL interfaces in test structures with varying InAs QD coverage. Unbalanced strain in the same structures shows a distortion in the GaAs peaks.

Walnuts contain a number of potentially neuroprotective compounds like vitamin E, folate, melatonin, several antioxidative polyphenols and significant amounts of n-3 α-linolenic fatty acid. The present study sought to determine the effect of walnuts on verbal and non-verbal reasoning, memory and mood. A total of sixty-four college students were randomly assigned to two treatment sequences in a crossover fashion: walnuts–placebo or placebo–walnuts. Baseline data were collected for non-verbal reasoning, verbal reasoning, memory and mood states. Data were collected again after 8 weeks of intervention. After 6 weeks of washout, the intervention groups followed the diets in reverse order. Data were collected once more at the end of the 8-week intervention period. No significant increases were detected for mood, non-verbal reasoning or memory on the walnut-supplemented diet. However, inferential verbal reasoning increased significantly by 11·2 %, indicating a medium effect size (P = 0·009; d = 0·567). In young, healthy, normal adults, walnuts do not appear to improve memory, mood or non-verbal reasoning abilities. However, walnuts may have the ability to increase inferential reasoning.

Improving the production of photocurrent in the middle junction of a InGaP/Ga(In)As/Ge triple-junction solar cells (TJSC) can improve the overall conversion efficiency of cell. One possible method to improve the middle junction photocurrent is inserting a quantum dot (QD) superlattice (SL) into the stack. It has been predicted that QD-SL enhanced TJSCs have an efficiency ceiling of 47% under a one-sun AM0 illumination spectrum. Additionally, QD array enhanced GaAs cells have the added benefit of possible intermediate band effects, anisotropic absorption and enhanced radiation tolerance. Embedding InAs quantum dots (QDs) in a single junction GaAs solar cell can increase sub-GaAs bandgap photocurrent generation. This method has been shown to improve the short circuit current density (Jsc) of single junction cells under simulated 1 sun air mass zero (AM0) illumination. However, the increase in strain due to the InAs QD self-assembly may cause defects that reduce the minority carrier lifetime resulting in losses in the cell open circuit voltage (Voc) on the order of 300-500 mV. The introduction of strain compensation (SC) layers into the superlattice (SL) structure of a QD solar cell has previously been shown to improve the device performance, including the partial recovery of Voc. Strain compensation can be used effectively to balance the residual strain, impede dislocation formation, and improve the solar cell characteristics. The effect of GaP strain compensation on the solar cell electrical and material properties was investigated. High resolution X-ray diffraction (HRXRD) scans along the symmetric (004) Bragg peak show weak intensity and wide FWHM at the zero order SL peak (SL0) for non-SC samples. Optimum SC thickness was theoretically determined using a zero in plane stress method and experimentally verified using HRXRD. Optimal strain compensation was then used to increase the QD SL stacking from 5x to 10x and 20x. Use of SC resulted in shifting of the SL0 peak toward the substrate peak as well as reduced FWHM and improved SL peak definition. Kinematical diffraction modeling of the QD structures using numerical simulation indicated this peak shift resulted from reduced overall strain in the SL stack up to 5ML of SC. The material quality improvement in the SC QD solar cells was manifested in an improved spectral response and Jsc. The optoelectronic results for GaAs solar cells with QD SL’s demonstrate a strong dependence on GaP SC layer thickness. In addition, comparison of multi junction (MJ) solar cells which incorporate the SC QD SL’s demonstrate the utility of additional sub-GaAs bandgap current contribution as a tool for additional current-matching optimization in MJ solar cells.

UK Nirex has supported a programme of work to develop models describing the postclosure evolution of intermediate-level waste packages with the objectives of:
• providing support and justification for the parameters and representations used in performance assessment models;
• informing future model development and packaging advice.
Scenarios for the potential evolution of a waste package were developed and modelled taking explicit account of waste package heterogeneity and the time-dependence of the physical and chemical characteristics of the system. The modelling work highlighted the treatment of organic complexants and the representation of physical containment as two areas in which the impacts of time dependence and package scale heterogeneity might be particularly significant.
A subsequent study of the impact of organic complexants emphasised the importance of heterogeneity in package inventory in determining the radionuclide release from the near field.
The degree of containment afforded by the wasteform and the waste container has been investigated as part of a study to develop a preliminary understanding of the mixing scales within the repository. The study suggests that the most important control on the release of radionuclides from the waste packages is the integrity of the waste encapsulation grout. Interactions between neighbouring packages are to be expected, but the degree to which homogeneous (well mixed) conditions develop may be limited in both time and space.

Sol-gel materials of TiO2 and vanadium-modified TiO2 of nominal composition 4, 8, and 16 wt.% vanadium were prepared by evaporation of aqueous colloidal sols obtained by the hydrolysis of aqueous solutions of titanium(IV) chloride with the appropriate amount of vanadyl oxalate using aqueous ammonia followed by peptization of the resulting hydrated solids using nitric acid. The nature of the sol-gel materials and their behavior on calcinations at temperatures up to 1273 K were investigated using x-ray fluorescence, powder x-ray diffraction, transmission electron microscopy, and electron diffraction and FT-Raman spectroscopy. At 333 K, all the gels comprised small (about 5 ± 1 nm) particles of anatase together with traces of brookite. The particle size changed little on thermal treatment at 573 K, but increased significantly at higher temperatures and was accompanied by transformation to rutile. Incorporation of vanadium in the gels reduced the temperature at which rutile began to appear from 923 K in pure TiO2 to 773 K in the V/TiO2 gels. Only rutile was present at high temperatures, except for the 16 V/TiO2 gel, when small amounts of phase-separated vanadia were also observed. A 2–3% substitutional incorporation of V4+ ions in the tetragonal rutile lattice occurred at high temperatures, but the majority of the vanadium was present in an amorphous, highly dispersed fashion.

Micellar calcium phosphate has the chemical composition and physicochemical properties that are consistent with it being a complex of the phosphate centres of casein with an acidic amorphous calcium phosphate. Similar acidic amorphous calcium phosphates have been prepared in the laboratory and for these, as well as for micellar calcium phosphate, the most appropriate crystalline model compound from which the short-range structure may be derived is brushite, CaHPO4.2H2O. The predicted secondary structures around sites of phosphorylation in the Ca2+-sensitive caseins often comprise an α-hclix-loop-α-helix motif with the sites of phosphorylation in the loop region. This motif may be important in linking the colloidal calcium phosphate with casein in native casein micelles.
